Mint a telemetry stream URL

Mints a fresh telemetry_url for an active session — the same read-only WebSocket URL POST /phones:allocate returns once (live trace spans + output logs for exactly this session). This is the telemetry/frames leg, distinct from the live-view (video) token: it can only watch the trace, never the screen, and never drive the phone. The stream’s end frame is the session-end signal. Refused for inactive sessions — an ended session’s telemetry is served by GET /phones/sessions//frames — and for sessions outside the caller’s organization (reads as not found).
